Management of patients hospitalised due to COVID-19 OR with hospital onset COVID and a new oxygen requirement

 

 

Why is a blueteq form needed?

  • To ensure treatment(s) are prescribed in line with the commissioning position i.e. patients are suitable for treatment 
  • This data is shared with NHS England to review usage nationally
  • To re-claim the cost spent on treatments 

What do you need to do?

  • Ensure any prescribed treatment(s) are in line with the LTHT COVID guidelines 
  • Complete the blueteq form for the relevant treatment(s) 
  • Upload to the patient’s PPM+ record as a PDF document
  • Inform pharmacy so the appropriate stock can be issued 

Blueteq access

  • If you have blueteq access we encourage you to complete the forms directly onto the system.
  • If you would like to get access to the blueteq system and complete the forms directly onto the system, see below:

Step 1: Register with Blueteq site: Hi-Cost Drugs Database Trust Edition - National (blueteq-secure.co.uk)
Step 2. If you can't access the full Blueteq system to find/complete Blueteq forms, you'll need to go through You go through the Trust's DIT portal on the intranet: Requesting IT support - Leeds Teaching Hospitals NHS Trust (leedsth.nhs.uk)

Document in the clinical notes that you have submitted a form onto the blueteq system, if you complete the forms directly onto the system.
